Rehabilitation services
Rehabilitation services are designed to relieve pain or discomfort from an injury or illness; improve the function, strength and mobility of individuals battling a disability or debilitating disease; and help restore physical, cognitive and neurological abilities impaired by trauma or illness.
We use a hands-on approach to help patients attain the most functional level of independence possible. Our specially-trained team of rehabilitation professionals works diligently to provide customized treatment programs for each patient. We provide inpatient and outpatient services, with a variety of treatments and specialized care.

For patients with a prescription for outpatient therapy, please print and fill out the Past Medical History form and Outpatient Therapy Summary List before your appointment. Be sure to wear comfortable, loose-fitting clothes and arrive early.
